Psychiatrist

Master level provider (completed degree) in one of the above professions.

Second year master/graduate student in one of the above professions (Proof of current school registration may be requested. If you cannot provide this information, you will not be able to attend and will not be eligible for a refund).

Confidentiality

I agree to keep confidential the personal identifying information of case material shared in the training.

I agree not to record any part of this training in any format (audio, video, or visual); my personal written notes of non-confidential material are permitted.

I accept that recording any portion of this training may result in me being asked to leave immediately without refund or compensation.

Conduct/Professional Behavior

I agree to maintain professional behavior that is expected of mental health providers. I accept that behaving in an unprofessional manner, including engaging in discriminative behaviors, disruptive behaviors, inappropriate comments or actions, etc. will result in immediate removal from this training without a refund or compensation.
